USCIS has updated the form it uses for medical exams. It is important that your civil surgeon uses the newly updated form when they complete your medical exam to help ensure your case has the correct form and will be accepted by USCIS. Please ask your civil surgeon to use the 01/20/25 edition date of Form I-693.

The Lisinski Law Firm represents the individual appearing at this medical exam in their application before USCIS. USCIS has recently updated the form (Form I-693) it uses for medical exams. It is very important for our client’s application that you use the 1/20/25 edition of Form I-693 when completing the medical exam today. Otherwise, USCIS might reject the application.
The edition date is located at the bottom of each page of the I-693 form.
