A Civil Surgeon is a doctor approved by USCIS specifically for immigration-related medical exams. This is different from your regular doctor, and you must visit a USCIS-approved Civil Surgeon.

Complete the Medical Examination

At your appointment, the Civil Surgeon will complete Form I-693. They will provide you with a sealed envelope containing the completed form. Do not open this envelope.
Mail or Drop Off the Sealed Envelope

Your case cannot be submitted to USCIS without this form. Include your full name on both the envelope and the packet itself to ensure accurate processing by our team.

What to Bring to Your Medical Appointment:

Government-Issued Photo ID: Valid passport, driver’s license, or other acceptable form of identification.
Vaccination or Immunization Records: Ensure your records are up to date (e.g., COVID-19, hepatitis A & B, MMR, etc.).
Health Insurance Card: Check if the Civil Surgeon accepts your insurance. Note that some plans may not cover all parts of this examination.
Payment: Fees typically range from $300–$600. Confirm pricing and payment methods with the Civil Surgeon before your appointment.
